Two or more questions should identify the reason for the request or the item the request is about.
Use as many different answer types as possible.
Once you are done, open the service item (using open anonymous) and try submitting.
Things to Think About
If notifications depend on certain fields – make them required.
The required field check is not made until the page is submitted. A handy visual cue such as bolding the label or an asterisk in the label (or both) will help your customers make fewer mistakes as they complete a request.
